Hazard Recognition Through Hazard Control Models (Four-M or Goal Accomplishment Model)
For this activity, complete the following:

Choose an injury or fatality investigation report from one of the following sources:

U.S. Chemical Safety Board (Links to an external site.)

Read the report.
After reading the report, write a paper that addresses the following requirements:

Present a brief summary of the event and highlight the losses associated with the incident (fatalities, injuries, costs, social consequences, etc.).
Provide an overview of the hazards that contributed to the incident. You should use and apply this information to one of the models from the book (Four M’s or the Goal Accomplishment Model). In other words, present the aspects of the model and identify the hazards associated with those aspects.
Present information as to how the incident could have been prevented through applying the model you chose. Be sure to focus on hazard control and apply information from the text and readings.
Lastly, provide a discussion defending hazard control as a primary role and responsibility of today’s safety professional. If not evident in section 3, describe how a safety professional could have contributed to controlling the hazards that contributed to the incident.

Note: The paper should be presented in APA format. The paper should be approximately 4-5 pages, not including cover, abstract, references, etc.
